New Patient Registration
Patients living within the Practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill.
You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous Doctor.
You have the right to express a preference of practitioner. If you wish to exercise this right, please inform our reception staff who will ensure that this information is included in your registration details. Do bear in mind however, that you will be registering with the Practice rather than an individual GP.
All new patients are offered a health check with a member of the nursing team. Ths is important as it enables us to record full details of your medical history, take up-to-date blood pressure readings and discuss your general health with you. You will be asked for a urine sample for testing aswell as information about any family history of illness and chronic conditions.
Medical treatment is available from the date of registration. Please contact reception for further information.
